The Minto-Mapleton Family Health Team (FHT) consists of a group of health professionals working together with 9 primary care physicians and 4 nurse practitioners who have a combined patient roster of approximately 15,000 patients. While our corporate headquarters is located in Drayton, we also have offices in Palmerston, Clifford and Harriston and service the Minto and Mapleton communities.

Cognitive Behavioural Therapy (CBT) Group 


These groups are being offered virtually during the pandemic!

The social workers of the Minto-Mapleton FHT offer Cognitive Behavioural Therapy (CBT) groups throughout the year. Cognitive behavioural therapy explores the link between how we think and what we do and how this influences how we feel.  This is an 8 week group with an educational component.  Participants will be given the opportunity to share experiences and strategies with group participants.

Anxiety - Dial it Down

This support group, hosted by the East Wellington Family Health Team in partnership with Ontario Telemedicine Network (OTN), is offered through video conferencing as an opportunity to learn tips to help manage anxiety. Evening education from 6:30pm - 8:30 pm (locations vary). Offered several times per year.

Alzheimer Care-Giver Support Group

Call 519-742-8518 ext.2090 to register for virtual sessions during the pandemic.

This support group, hosted by the Alzheimer Society in partnership with Ontario Telemedicine Network (OTN), is offered through video conferencing, as an opportunity to connect with other caregivers. There is a brief presentation topic of the group’s choice, followed by group discussion. Offered in Palmerston and Mount Forest the first Tuesday of every month (except July and August) from 10am - 11:30am.

Mental Health/Counselling Services

Now more than ever, it is so important to manage on-going mental health concerns.  We are offering video and phone appointments, don't delay, call for help today!
The social workers offer psycho-education related to the prevention of mental health problems as well as individual, couple, family, and group counselling and psychotherapy.

Specific mental health concerns that the social workers can help with include:

  • Depression, including post-partum depression
  • Anxiety Disorders/Worry
  • Self-Esteem
  • Stress Management
  • Parenting Concerns
  • Coping with Illness
  • Couple and Family Problems
  • Grief


Activity Counselling 

Virtual support for home based exercise plans & programs are available during the pandemic.  Reach out to us!

Active people are happier and healthier.  Adding more regular exercise to your life will give you increased energy, make you feel better and improve your quality of life. Regardless of what stage you are at with your exercise program, our Registered Kinesiologist can help you determine your current fitness level and create an individualized program aimed at improving your health.


Community Outreach

Our outreach workers are still here to help during this pandemic.

The Rural Wellington Community Team understands that it's hard to reach out and find the resources you need, when you need them. We work in the community and with each rural health team to connect you to community agencies, help you to navigate social programs, and support you when family or friends can’t help.


Geriatric Psychiatry

Virtual care continues with this service during the pandemic.
The Minto-Mapleton Family Health Team offers Geriatric Psychiatry Services via video conferencing from our Clifford, Harriston, Palmerston and Drayton locations.

Geriatric Psychiatrist work with our Registered Nurse, Pam Turton, to provide assessments for patients 65 years and older, or for any age-related concerns with any of the following criteria:

  • Behavioural difficulties
  • Cognitive decline / Memory loss / Dementia
  • Mood disorder, anxiety, psychosis
  • Psychiatric medication review
  • Substance misuse
