Specialized Outreach Services (SOS) - Stonehenge Therapeutic Community
The S.O.S. program is an integrated team of peers and professionals that offer flexible outreach services to individuals experiencing homelessness or street-involved individuals with addiction, mental health, or concurrent issues by providing supportive nursing and counselling, connections to primary care, and referrals to other community services. Services are provided to individuals living in Kitchener-Waterloo, Guelph, Cambridge, and surrounding areas.
Program Criteria
The S.O.S. program is available to individuals who are:
- 18 years of age or older
- Disconnected from services, and
- Street-involved, homeless or at risk of homelessness, and
- Living with mental health/addiction/concurrent issues, and
- Experiencing physical health issues
Referral Process
The primary point of access for this service is through outreach connections established on the street, in shelters, at food banks, soup kitchens, etc. Referrals can also be made by contacting team members directly by calling 519-212-7824 in Guelph.
