VON offers a NO COST, in-home support services to promote independence and improve wellness and quality of life.

Senior Support Workers provide a wide range of services and coordinating, including:

• Support to get back on your feet following a hospital discharge or illness
• Temporary crisis intervention with housekeeping, meal preparation, laundry, safety checks, etc
• Resourcing for seniors with questions or health concerns
• Referring to formal supports
• Linking to community supports (ex. finding a Doctor, Meals on Wheels, Homemaking)
• Assistance with the completion of form
• Providing opportunities for socialization and recreation
• Visiting at-risk seniors, providing exposure to supports available
• Accompaniment to appointments, offering comfort/companionship/advocacy
• Assisting with picking up groceries, or supplies when ill
• Providing urgent respite to caregiver

Eligibility for the Program:

65 years of age or older, or older adults with a physical disability
Service is short-term in nature rather than on an ongoing basis
No professional referral required
